Today, let’s explore how four critical lifestyle factors—smoking, physical activity, alcohol consumption, and stress — directly impact your long-term risk of developing cataracts.
1. Smoking
When you smoke cigarettes, you introduce toxins that damage the vital internal structures necessary for maintaining clear vision. This damage can cloud your lenses (cataracts), constrict your field of vision (from glaucoma), or lead to central vision loss (macular degeneration).
Because of this, people who smoke are two to three times more likely to develop cataracts. While most cataracts develop slowly over years as we age, smoking accelerates this timeline, creating a 40–66% higher risk of all cataract types for anyone who has ever smoked.
Other ocular risks associated with smoking include:
- 160% higher risk of Posterior Subcapsular cataracts (PSC) over 10 years
- 143% higher risk of uveitis (painful ocular inflammation)
- 135% higher risk of retinal vascular occlusions (artery or vein blockage)
- 57–147% higher risk of glaucoma
- 85% higher risk of age-related macular degeneration (AMD)
- 21% higher risk of diabetic retinopathy
Ongoing data shows that opting for Nicotine Replacement Therapy (NRT) is linked to a short-term lower risk of cataracts, glaucoma, and macular degeneration compared to continuing to smoke cigarettes. Talk to your primary care physician if you are a smoker and need help quitting.
2. Physical Activity
Did you know that physical activity has a protective role to help delay cataract formation too? Your eye’s lens is uniquely vulnerable to oxidative damage. Over time, an accumulation of toxic reactive oxygen species disturbs the lens’s fundamental structure, breaking down crystallin proteins and creating visible opacities (cataracts). Exercise boosts the body’s natural antioxidant enzymes leading to less cell injury and reduces the body’s inflammatory cascade.
What the Numbers Show:
- Regular physical activity is associated with a 13–24% risk reduction for cataracts.
- Conversely, prolonged periods of daily inactivity (sitting for more than 6 hours) are linked to a 27% higher cataract risk.
- Both running and brisk walking lead to a 16.4% lower risk of cataracts. “Moderate activity” means working out to the point where you can comfortably talk, but cannot sing.
- For those living with diabetes, exercising more than 14 times per week (compared to less than 5 times) decreased the risk of needing cataract surgery by 19% over an 8-year period.
Aim for 60 minutes of daily movement to support your vision healthspan!
3. Alcohol Consumption
When it comes to alcohol, quantity and type matter immensely. Data shows a distinct contrast between mild and heavy drinking habits:
- The benefit of moderation: Compared to complete abstinence, mild to moderate wine consumption (1–2 drinks per day, up to 4 times a week) is associated with a 14–23% lower risk of needing cataract surgery. Red wine confers the greatest protective benefit, likely because of the high concentration of protective polyphenol antioxidants found natively in red grape skins.
- The danger of excess: This benefit completely reverses with heavy or cumulative drinking. Heavy drinkers show a 5–6% higher risk of needing cataract surgery than moderate drinkers. Over a lifetime, drinking more than 4–7 drinks per week results in a 30% higher risk of developing cataracts for both men and women.
4. Stress
While a definitive direct link between psychological stress and cataracts is still being studied, we know that chronic stress floods the body with inflammation—a known contributor to earlier cataract formation.
However, the reverse connection is already documented: cataracts profoundly impact your mental well-being. Because cataracts affect your vision and how you see the world, they are associated with 70–87% higher risk of developing depression and dementia.
Undergoing timely cataract surgery reduces the risk of developing depression and dementia by 25% simply by restoring visual clarity and reconnecting you with your surroundings. Currently, we only have surgery as the main treatment for visually significant cataracts.
