September is Healthy Aging Month! I’ve been recommending high-quality vitamins for a long time, but one thing that struck me at the Nutrition in Medicine conference was the landmark COSMOS (COcoa Supplement and Multivitamin Outcomes Study).
The findings suggest that something as simple and accessible as a daily multivitamin may have a role in supporting memory and cognitive health as we age.
Let’s explore what the research actually shows.
What Is the COSMOS?
COSMOS is a large randomized clinical trial involving more than 21,000 adults age 60 and older. Researchers have been studying whether daily supplementation with a multivitamin-mineral supplement and/or cocoa extract can influence health outcomes, including cardiovascular health, cancer, and cognition.
The cognitive findings have been particularly interesting because researchers have examined cognition from several different perspectives and multiple studies within COSMOS have produced complementary findings.
Could a Multivitamin Slow Cognitive Aging?
In the COSMOS-Mind study, more than 2,200 older adults were followed for three years.
Participants taking a daily multivitamin-mineral supplement demonstrated a statistically significant improvement in global cognition, along with benefits in memory and executive function, compared with those taking a placebo. The benefit appeared more pronounced among participants with a history of cardiovascular disease, although this subgroup finding needs further study.
Researchers then used statistical modeling to put the size of the effect into perspective. Based on the study’s cognitive-age model, three years of multivitamin supplementation appeared to correspond to about 1.8 years or approximately 60% of slowed cognitive aging.
What About Memory Specifically?
A separate COSMOS study, COSMOS-Web, included more than 3,500 adults age 60 and older who completed computerized cognitive testing over three years.
The results were encouraging: people assigned to the daily multivitamin had better immediate recall after one year, the study’s primary outcome, and the memory benefit remained evident across the three-year follow-up. Researchers estimated that the improvement was equivalent to approximately 3.1 years of age-related memory change.
That’s an important finding because memory is one of the cognitive abilities many people notice changing as they get older.
Does That Mean Multivitamins Prevent Dementia?
Not necessarily and this distinction matters.
The COSMOS-Mind researchers specifically looked at whether multivitamin supplementation reduced the incidence of mild cognitive impairment (MCI) and dementia over three years.
They did not find a statistically significant reduction in the number of new MCI or dementia diagnoses. However, among participants who did develop MCI, those taking the multivitamin had higher global cognition and executive-function scores and showed less decline during the year before diagnosis. Researchers described this as a potential form of “cognitive resilience.”
In other words, the research does not show that a multivitamin prevents dementia. It does suggest that adequate nutritional support may help the brain maintain cognitive function and buffer against decline.
And the Evidence Keeps Building
The COSMOS research has now been examined across multiple cognitive studies.
A 2024 analysis combining data from COSMOS-Mind, COSMOS-Web, and COSMOS-Clinic found evidence of benefits in both global cognition and episodic memory. The researchers estimated that the effect on global cognition was equivalent to approximately two years less cognitive aging.
Having several related studies within the same large research program point in a similar direction makes the findings particularly interesting while still leaving important questions about exactly which nutrients are responsible and who benefits most.
Nutrition Still Comes First
A multivitamin shouldn’t replace a healthy diet.
The brain needs a wide range of vitamins, minerals, healthy fats, protein, and other nutrients to function properly. A nutrient-dense diet, regular physical activity, restorative sleep, meaningful social connection, and ongoing mental engagement all remain important pieces of healthy aging.
In fact, researchers have emphasized that multivitamins should be considered as part of a broader healthy lifestyle not as a replacement for one.
A Personalized Approach Matters
Multivitamins are generally well tolerated when appropriately formulated and taken as directed, but more is not necessarily better.
Taking excessive amounts of certain nutrients particularly fat-soluble vitamins such as vitamins A, D, E, and K can lead to toxicity. Supplements can also interact with medications or other supplements.
Because everyone’s baseline needs differ, nutritional support should always be personalized.
Nutrient Revolution: Supporting Your Eyes & Whole-Body Health
Supporting overall health can be an important part of supporting long-term vision health. One nutritional option I recommend is Nutrient Revolution’s Macular Focus® + Multivitamin.
This formula combines the clinically studied antioxidants and carotenoids found in Macular Focus® with a broader spectrum of vitamins and minerals to provide nutritional support for retinal and whole-body health.

The Bigger Picture
The COSMOS findings don’t mean that taking a multivitamin guarantees better memory or prevents dementia.
But they do raise an important question:
Could something as simple as making sure we’re meeting our nutritional needs help support the brain as we grow older?
The research so far is reassuring, particularly for memory and overall cognitive function, but additional research is needed to understand the long-term effects, the specific nutrients responsible, and which individuals may benefit most.
Healthy aging isn’t about finding one magic pill.
It’s about giving your body and your brain the nutritional support, movement, sleep, connection, and care they need to thrive.
